Mississippi Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and recommend care management plans to improve healthcare outcomes
• Develop ongoing care plans in collaboration with providers and facilitate access to necessary resources
• Monitor member progress and update care plans as needed to meet individual goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse licensure required in Mississippi or compact state
• Experience in developing and implementing care plans
• Familiarity with healthcare regulations and compliance standards
