Job Summary

A company is looking for a Triage Registered Nurse - Home Health Remote PT Weekend Variable.

Key Responsibilities

• Collect and analyze healthcare data from telephonic contacts and EMR information
• Develop and implement care plans, coordinating care delivery and evaluating patient outcomes
• Delegate care tasks to appropriate healthcare workers while promoting quality patient outcomes

Required Qualifications, Training, and Education

• Graduate from an accredited School of Nursing; BSN preferred
• Current RN license or temporary license in the state of practice
• Basic Life Support for Healthcare Provider certification from the American Heart Association
• Additional education or certifications may be required by the nurse leader
• Ability to meet physical demands of the role, including lifting and responding to patient needs
