A company is looking for a Care Manager Transition of Care.

Key Responsibilities
  • Evaluate member needs through post-discharge assessments and medication reconciliation
  • Develop care/service plans in collaboration with healthcare teams to support member transitions
  • Facilitate education and resources to promote wellness and improve overall quality of care
Required Qualifications, Training, and Education
  • Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
  • 2 - 4 years of related experience
  • Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
  • North Carolina residency required
  • RN preferred
