New York Licensed Case Planner

Job Summary

A company is looking for a Case Planner II to support families in crisis within their Preventive Services Program.

Key Responsibilities:

• Provide individual and family counseling to support family stability
• Conduct clinical assessments and develop intervention plans
• Monitor progress and provide therapeutic support under the supervision of an LCSW-R

Qualifications:

• Master's degree in Social Work (MSW), Mental Health Counseling (MHC), or Marriage and Family Therapy (MFT) from an accredited program
• Experience or knowledge in therapeutic modalities (individual, family, group)
• Understanding of trauma-informed care and child welfare systems
• Must demonstrate the ability to work with culturally diverse populations
• Spanish fluency highly preferred
