New Jersey Licensed Case Manager

Job Summary

A company is looking for a Care Connect Case Manager (REMOTE) - Oncology.

Key Responsibilities:

• Assess patients' clinical needs to ensure appropriate care levels and lengths of stay
• Coordinate and implement care plans while monitoring services and advocating for members and their families
• Serve as a mentor/trainer for new staff and act as a subject matter expert in various projects

Required Qualifications:

• High School Diploma/GED required; Bachelor's degree preferred or relevant experience in lieu of degree
• Minimum of two (2) years clinical experience required
• At least two (2) years of healthcare payer experience required
• Active Unrestricted NJ RN License or active Compact License required
• CCM certification preferred for Clinical Advocate roles
