Job Summary

A company is looking for an LPN Weekend Triage/Scheduling professional for home care services.

Key Responsibilities

• Conduct patient phone triage and schedule caregivers
• Manage inbound and outbound calls while providing skilled nursing services under RN supervision
• Assess, plan, implement, and evaluate patient needs and care

Required Qualifications, Training, and Education

• Graduate of an accredited practical nursing program with a current LPN license
• IV Certification within 1 year of hire
• Must possess a valid driver's license
• At least 1 year of nursing experience
• CPR certification required
