Job Summary

A company is looking for a Social Care Navigator to support members facing social, behavioral, or environmental challenges impacting their health.

Key Responsibilities

• Apply motivational interviewing techniques to help members set and achieve personal health and wellness goals
• Proactively reach out to members to assess needs and provide tailored support
• Guide members to appropriate local programs and resources for assistance

Required Qualifications

• 2+ years of experience in Care Navigation, Social Work, Health Coaching, or similar roles; Community Health Worker certification is a plus
• Proficiency in Motivational Interviewing or similar methods
• Understanding of systems like Medicaid, SNAP, and public housing
• Experience working with low-income individuals or those with chronic conditions
• Ability to work independently in a remote environment and utilize digital platforms for documentation
