Maine Licensed RN Case Manager

Job Summary

A company is looking for an RN Case Manager - Remote.

Key Responsibilities:

• Coordinates patient-centered care and ensures safe discharges through collaboration with patients, caregivers, and the healthcare team
• Manages utilization and compliance by applying knowledge of regulations and supports appropriate length of stay and reimbursement
• Develops and implements discharge plans for complex cases and facilitates timely transitions to the next level of care

Required Qualifications:

• Associate's degree (or higher) in Nursing
• Current, unrestricted RN license in the state of residence
• 3+ years of experience in a hospital, acute care, or direct care setting
• Intermediate level of proficiency to type and navigate a Windows-based environment
