Licensed Care Management Nurse

Job Summary

A company is looking for a Care Management Nurse to support patients on their journey to better health and well-being.

Key Responsibilities

• Create and promote adherence to treatment plans developed by healthcare providers
• Monitor and document patient progress, ensuring continuity of care through relationships with providers and community resources
• Assess patients' unmet health and social needs while maintaining confidentiality and compliance with HIPAA regulations

Required Qualifications

• Current standing as a licensed practical nurse (LPN)
• 5 years of experience in a clinical or community resource setting/care coordination
• Case Management experience is preferred
• Highly organized with the ability to maintain accurate notes and records
• Proficient computing skills and ability to learn new systems
