Job Summary

A company is looking for a Health Navigator, Member Outreach.

Key Responsibilities
  • Conduct educational telephone calls to advise members on available benefits and services, completing health needs assessments and referring to appropriate programs
  • Reach out to members with gaps in care, encouraging compliance and assisting with provider location and appointment scheduling
  • Manage system work queues, screening members for eligibility and assigning them to clinical teams for outreach and intervention
Required Qualifications
  • Associates Degree in education, communication, or health-related field or equivalent work experience
  • 2 years of experience in a customer service support role is required
  • Ability to work independently with minimal supervision
  • Knowledge in medical terminology
  • Proficient interpersonal and organizational skills
