Georgia Licensed RN Care Coordinator

Job Summary

A company is looking for an RN Care Coordinator, REMOTE.

Key Responsibilities

• Implement interdisciplinary processes to evaluate healthcare options and services for patients
• Conduct initial and concurrent chart reviews for utilization review and identify patients needing case management follow-up
• Facilitate discharge planning and communicate with external agencies for smooth transitions from hospital to post-hospital settings

Required Qualifications, Training, and Education

• Current GA RN license
• BLS certification required within the first 90 days of employment
• Graduate of an accredited school of nursing; BSN recommended
• Minimum of 2 years nursing experience, preferably in a hospital setting
• At least 1 year of experience in case management, utilization management, or discharge planning preferred
