Clearwaters.IT is in search of a Regional Care Coordinator who will be providing comprehensive clinical care coordination support.
This role requires highly organized, proactive professionals with excellent communication skills and a deep understanding of patient care coordination within a military or federal healthcare system
This position is 100% on site in the following location:
Walter Reed National Military Medical Center, Bethesda, MD
Responsibilities:
Collaborate with the national care coordinator to seek guidance on coordination processes, apply best practices, and ensure regional alignment with the MHS-wide clinical care coordination program.
Reaching out to patients regarding their medical care appointments, including scheduled, pending, missed, or potential visits, and will provide them with necessary administrative and health-related instructions.
Manage the scheduling of initial exams, procedures, surgeries, and follow-up visits, ensuring seamless coordination of pre-operative and post-operative care for DoD beneficiaries, VA patients approved for MHS care, and civilians authorized by the DoD.
Assist patients in understanding and navigating base access procedures and liaise with base personnel to facilitate entry for scheduled care.
Support providers by discussing care plans with patients and their caregivers, scheduling appropriate follow-up, and assisting with pre-authorization of medications and the procurement of specialized treatments such as tears, surgical tissue, or off-label medications.
Track the progress of individuals from initial diagnosis or injury through all phases of treatment and rehabilitation, including transitions between the DoD, VA, and civilian providers.
Monitoring and managing TRICARE and VA authorizations or extensions will be essential to ensuring uninterrupted care.
Oversee the referral and consultation process across the DoD, VA, and community care networks, working to streamline communication, reduce delays, and build lasting relationships between referring providers.
Track and analyze clinical outcomes and support special-interest patient populations.
Document all relevant patient encounters will be completed in the DoD medical record system according to established protocols and under COR guidance.
Gather data on patient satisfaction, care quality, and quality of life will also be a key responsibility, with data collected through various channels including direct interaction, phone calls, surveys, and electronic means.
Must be a U.S. Citizen
Ability to obtain and maintain appropriate base access and any security clearance (Public Trust) required by the DoD
Minimum of five (5) years of clinical experience in a relevant role such as healthcare coordinator, social worker, case manager, or registered nurse.
Proficiency in electronic medical records (EMR) DOD/VA systems
Excellent verbal and written communication skills for patient engagement, provider collaboration, and data reporting.
