Job Summary

A company is looking for an Analyst, Case Management - Work From Home.

Key Responsibilities
  • Assess members' needs and document interactions in electronic records to ensure compliance
  • Connect members to community and national resources, and provide education on available services
  • Support members during enrollment calls and assist in problem-solving when services are unavailable
Required Qualifications
  • At least 2 years of experience in healthcare, nonprofit, or community-based services
  • Experience with high call volume and auto-dialer systems
  • Preferred experience in information & referral, health coaching, care management, or social work
  • Bachelor's degree or equivalent work experience
  • Bilingual (Spanish) preferred
