Job Summary

A company is looking for a Care Coordinator to support members through telephonic interactions and collaboration with clinicians.

Key Responsibilities:

• Handle inbound and outbound calls to support members
• Complete intake screening questionnaires and audio/video initial program assessments with members
• Conduct cold calls for member engagement and assist with scheduling appointments

Required Qualifications:

• 2+ years of healthcare/clinical experience
• Call agent experience is preferred
• Basic understanding of EHR/EMR applications
• Proficiency in Microsoft Office
• Must live in the United States
