Job Summary

A company is looking for a Remote LPN - Chronic Care Management.

Key Responsibilities

• Provides direct patient care as per the patient's plan and consults with healthcare professionals as needed
• Performs therapeutic procedures and accurately observes, records, and reports patient conditions
• Assists in the development of patient care plans and educates patients and families on care and rehabilitation

Required Qualifications

• Current Licensed Practical Nurse license in the applicable state or multi-state through the eNLC
• Certification in Basic Life Support within 30 days of hire
• One year of clinical experience preferred
• Completion of annual required education related to specialty accreditation, if applicable
• Ability to meet physical demands, including lifting and moving patients and equipment
