Job Summary

A company is looking for an RN Case Manager to coordinate patient care and advocate for patients across various healthcare settings.

Key Responsibilities

• Conduct intake calls and ongoing patient outreach to monitor care plans and address new concerns
• Provide care coordination for complex patients and develop plans of care with the care team
• Support patients during care transitions and collaborate with multidisciplinary teams to ensure comprehensive care

Required Qualifications

• Current RN License in good standing in the state of Washington
• Minimum associate degree in nursing required, bachelor's degree preferred
• Minimum of 3 years' experience in a clinical setting, with at least one year in case management
• Certified Case Manager (CCM) certification or commitment to complete when eligible
• Geriatric care experience highly desired
