Job Summary

A company is looking for a Case Manager - Registered Nurse - Telehealth.

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities for members to evaluate their medical needs
• Utilize clinical tools to conduct evaluations and apply clinical judgment to address health and social indicators impacting care planning
• Collaborate with stakeholders to overcome barriers and enhance member engagement through motivational interviewing

Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 1+ year of medical case management, care coordination, or discharge planning experience
• 3+ years of experience with personal computers and MS Office Suite applications
• High-speed internet access for telework
