Job Summary

A company is looking for a Social Care Navigator (Mandarin/Cantonese Speaking).

Key Responsibilities

• Apply motivational interviewing techniques to support members in achieving health and well-being goals
• Proactively reach out to members for needs assessment and provide tailored support
• Guide members to local programs and resources while ensuring consistent support through collaboration with internal teams

Required Qualifications

• 2+ years of experience in Care Navigation, Social Work, Health Coaching, or similar roles; Community Health Worker certification is a bonus
• Fluency in Mandarin or Cantonese is required
• Proficiency in Motivational Interviewing or similar change guidance methods
• Understanding of systems like Medicaid, SNAP, and public housing
• Experience working with low-income individuals or those managing chronic conditions
