Job Summary

A company is looking for a Clinical Pharmacist - Call Center - TX Licensed - Remote.

Key Responsibilities

• Conduct telephonic medication consultations to meet/exceed production and quality metrics
• Complete annual medication reviews and pharmacotherapy assessments for patients
• Deliver patient-centered medication consultations and document clinical consultations in the internal database

Required Qualifications

• Doctor of Pharmacy degree or equivalent experience
• Active, unrestricted pharmacy license in the state of primary residence
• 3+ years of Clinical Pharmacy Practice experience
• 1+ years of Call Center experience
• Experience with Medicare STARs & HEDIS Measures
