State Licensed Integrated Care Nurse

Job Summary

A company is looking for an Integrated Care Management Nurse or Clinician.

Key Responsibilities

• Drive member behavior change and engage family members in care coordination
• Assess medical, behavioral, social, and health system domains using a holistic approach
• Conduct essential case management activities including assessment, planning, implementation, and evaluation

Required Qualifications

• Associate or Bachelor's Degree in Nursing or Master's degree in a Behavioral Health related field
• Current unrestricted RN license or independent clinical license (e.g., LCSW, LMHC)
• 3 years of case management or similar experience in health care
• Licensure or certification allowing independent assessment in health or human services
• Equivalent combination of education and experience may be considered
