RN Case Manager - Compact License

Job Summary

A company is looking for an RN Case Manager with a Compact License for a remote position.

Key Responsibilities

• Make outbound and receive inbound calls to assess members' health status and care gaps
• Complete assessments for clinical and social needs and manage the plan of care
• Collaborate with other departments and Primary Care Physicians regarding member needs

Required Qualifications

• Current, unrestricted RN compact license
• 3+ years of experience in a hospital, acute care, or direct care setting
• Basic experience with Microsoft Office (Word, Excel, Outlook)
• Designated workspace with access to high-speed internet
