RN Advanced Illness Case Manager

Job Summary

A company is looking for an Advanced Illness Complex Case Manager III.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized care plans for members
• Facilitate advanced care planning discussions and coordinate services across the healthcare continuum
• Provide personalized navigation and advocacy for members facing late-stage cancer diagnoses

Required Qualifications

• Associate or bachelor's degree in nursing
• 5+ years of clinical/acute care nursing experience focusing on advanced illness, hospice, or palliative care
• Experience with vulnerable and complex populations in clinical, home care, or telephonic environments
• Current, unrestricted RN license in the state of residence; Certified Case Manager (CCM) preferred
• Proficiency in electronic health records and care management software
