Required Qualifications, Training, and Education

• Must be a licensed practical nurse (LPN)
• Experience in a remote care setting
• Strong technology skills for using electronic health records
• Ability to conduct telephonic outreach and assessments
• Knowledge of wellness, nutrition, and goal setting
