Job Summary

A company is looking for a Case Manager - Registered Nurse - Telehealth.

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities for members
• Utilize clinical tools to evaluate members' needs and develop strategies to reduce risk factors
• Collaborate with interdisciplinary teams and stakeholders to address barriers in achieving health goals

Required Qualifications

• Registered Nurse with an unrestricted nursing license and the ability to be licensed in non-compact states
• 3+ years of clinical/nursing practice experience
• 1+ year of medical case management, care coordination, or discharge planning experience
• High-speed internet access and proficiency in MS Office Suite applications
• 3+ years of experience with personal computers and multi-systems navigation
