Job Summary
A company is looking for a Nurse Case Manager - Maternity.
Key Responsibilities
- Assess the needs of members and develop individualized care plans
- Monitor care plans and coordinate with providers for effective service delivery
- Educate members on their conditions, treatment plans, and available resources
Required Qualifications
- Registered Nurse (RN) with a current, unrestricted license
- Preferred college degree
- Three or more years of experience in a Case Management role
- Three or more years of clinical experience in OB/L&D or related area
- CCM certification required within specified timeframe
