Develops and facilitates complex care management activities and personalized care plans for members
Coordinates care between members, families, and providers to ensure adequate person-centered services
Monitors member progress and updates care plans as necessary to meet goals and address unmet needs
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience in care management
Registered Nurse (RN) State Licensure and/or Compact State Licensure required
Strong background in Case Management and Behavioral Health Management
