Job Summary

A company is looking for a Registered Dietitian Case Manager.

Key Responsibilities

• Contact targeted members for health promotion and develop individual care plans based on claims data
• Assist patients in accessing primary care, specialists, and necessary medical equipment
• Document patient interactions and maintain compliance with privacy regulations

Required Qualifications and Education

• Graduation from an accredited RN program and current RN license
• Bachelor's degree in nutrition, dietetics, or a related field; Master's degree preferred
• Minimum of 3 years of clinical nutrition experience
• Prior experience in case management or wellness program coordination
• Knowledge of medical claims and coding (ICD-10, CPT, HCPCS)
