Key Responsibilities

  • Collaborate with healthcare teams to address patient needs and optimize care delivery
Required Qualifications
  • Associate degree in nursing required; bachelor's degree preferred
  • Current RN License required; Compact License preferred
  • 3-5 years of experience in telephonic care management and skilled nursing facility care
  • Experience in kidney care and RAC-CT certification preferred
  • Strong understanding of skilled nursing facility operations and regulations preferred
