Post Acute Care Nurse Manager

Job Summary

A company is looking for a Nurse Care Manager - Post Acute Care (CST).

Key Responsibilities

• Manage overall care for a caseload of nephrology patients in skilled nursing facilities
• Facilitate transitions from skilled nursing facilities to home or community settings
• Collaborate with healthcare teams to address patient needs and optimize care delivery

Required Qualifications

• Associate degree in nursing required; bachelor's degree preferred
• Current RN License required; Compact License preferred
• 3-5 years of experience in telephonic care management and skilled nursing facility care
• Experience in kidney care and RAC-CT certification preferred
• Strong understanding of skilled nursing facility operations and regulations preferred
