Pennsylvania Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Case Manager Behavioral Health (Crisis Triage) to work remotely.

Key Responsibilities:

• Provides telephone triage, crisis intervention, and care navigation for at-risk members with mental health and/or substance use concerns
• Engages with members and families to assess needs and provide navigation to appropriate behavioral health services
• Matches members with suitable providers based on clinical assessment and facilitates care coordination

Qualifications:

• Active PA Registered Nurse License required; B.S.N. or LSW, LCSW, LPC preferred
• Minimum of three to five years post-licensure behavioral health clinical experience
• Experience in Utilization Review or Case Management preferred
• Knowledge of DSM 5 and ASAM
• Ability to work independently and adapt clinical assessment skills to telephonic interactions
