Job Summary

A company is looking for a Palliative Service Line Advanced Practice Provider (Remote).

Key Responsibilities

• Identify eligible patients for palliative services in collaboration with the primary care team
• Conduct patient and caregiver education on the goals and scope of palliative care
• Perform intake assessments and support appropriate documentation for transitions

Qualifications

• High emotional IQ
• 5+ years of experience in the healthcare field
• 2+ years of experience in palliative, hospice, or transitional care
• Knowledge of end-of-life care, chronic illness trajectories, and advance care planning
