Job Summary

A company is looking for a Patient Care Advocate to join a Managed Care Organization.

Key Responsibilities

• Engage with Medicaid members and PCP offices to address care gaps
• Ensure members receive quality care through collaborative teamwork
• Participate in virtual meetings and utilize communication tools for project completion

Required Qualifications

• Clear and active LPN Oregon license or a bachelor's degree
• 4+ years of clinical experience if LPN; 2+ years if holding a bachelor's degree
• HEDIS experience is highly preferred
• Experience in patient and member-facing roles
• Background in a medical office setting
