Job Summary

A company is looking for an LVN Case Manager - Disease Management.

Key Responsibilities

• Conducts clinical evaluations and assessments to determine case management eligibility and individual needs
• Develops and implements case management plans in collaboration with members, caregivers, and healthcare professionals
• Maintains a caseload for regular outreach, monitoring, and documentation of interventions and goal achievements

Required Qualifications

• Graduation from an accredited Licensed Vocational Nurse program
• Current unrestricted LVN license in Oregon
• 2+ years of recent clinical experience as an LVN/LPN
• 1+ years of experience in care management, disease management, or discharge planning
• Access to a dedicated workspace and high-speed internet connection
