Job Summary

A company is looking for a Care Coordinator.

Key Responsibilities

• Coordinate care and disease management for high-risk patients, focusing on outreach and optimal outcomes
• Conduct clinical assessments and monitor patient compliance with care plans
• Serve as a liaison between patients, families, and healthcare teams to advocate for patient needs

Required Qualifications

• Graduate from an accredited school of professional nursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) certification through American Heart Association (AHA)
• Three to five years of nursing experience
• Background in adult care, med/surg, pulmonary, inpatient, or ambulatory settings
