Job Summary

A company is looking for a Senior Care Manager (RN) to assess, plan, and implement care management activities for members.

Key Responsibilities

• Develop and continuously assess personalized care plans and collaborate with providers to address members' needs
• Coordinate care between members, families, and care providers to ensure appropriate services are delivered
• Monitor and update care plans based on member status and collaborate with stakeholders to meet care goals

Required Qualifications

• Active New York State Registered Nurse License
• Degree from an accredited school of nursing or a Bachelor's degree in nursing
• 4 to 6 years of related experience
• Experience in care management and collaboration with healthcare providers
• Knowledge of state and federal regulations related to healthcare services
