New York Licensed HCBS Coordinator

Job Summary

A company is looking for a Remote HCBS Service Coordinator to support individuals receiving Home and Community-Based Services.

Key Responsibilities

• Coordinate and monitor HCBS services for a remote caseload of clients
• Develop and update person-centered service plans
• Collaborate with clients, families, providers, and interdisciplinary teams via phone or video

Qualifications

• Bachelor's degree in social work, psychology, human services, or a related field
• Experience with behavioral health populations strongly preferred
• Familiarity with HCBS programs and case management is highly preferred
