Job Summary

A company is looking for an Inpatient Acute Nurse Care Manager.

Key Responsibilities

• Collaborate with healthcare team members to improve patient health and manage chronic conditions
• Educate patients and caregivers on healthcare management and coordinate services
• Develop and maintain individualized care plans while addressing barriers to service

Required Qualifications and Education

• Current RN license in the state of practice
• Two years of clinical nursing experience
• BSN must be obtained within three years of hire or promotion
• Experience in clinical care management or with third-party payers is preferred
• Knowledge of care management and health insurance products is beneficial
