Job Summary

A company is looking for a Care Manager - NICU.

Key Responsibilities

• Engage members and their support systems through assessments and care planning
• Coordinate care and services with community partners and social service agencies
• Document care coordination activities and evaluate member satisfaction

Required Qualifications and Education

• Nursing degree from an accredited program or a bachelor's degree in a health care field
• Preferred advanced degree associated with clinical licensure
• Minimum of three years of NICU experience in nursing, social work, or related fields
• Preferred three years of Medicaid and/or Medicare managed care experience
• Current unrestricted clinical license as a Registered Nurse, Social Worker, or Counselor
