Job Summary

A company is looking for a Health Coach.

Key Responsibilities:

• Establish health improvement plans based on member specifications
• Support customers in achieving personalized health goals through behavior changes
• Communicate results of reviews and ensure accurate documentation of clinical decisions

Required Qualifications:

• Associate's degree required
• LPN/LVN with a valid, current license issued by the state of residence
• Ability to complete assigned work in a timely and accurate manner
• Ability to work independently
• Experience in customer service is preferred
