The successful candidate will be responsible for overseeing the disease registry database, conducting pre-visit chart reviews, coordinating care across the continuum, and participating in quality improvement activities. This job requires a proactive approach to patient care and a commitment to continuous improvement in healthcare delivery. Responsibilities

Requirements
• Graduate of an accredited school of practical nursing.

• 2 years of ambulatory care experience required.

• Current licensed practical nurse (LPN) license in state of practice required.

• Proficiency in using computers, software, and web-based applications.

• Effective verbal and written communication skills.
,
• Ability to present information clearly and professionally.

• Highly motivated and eager to learn new skills.

• Strong organizational, decision-making, and delegating skills.

• High level of positive interpersonal and public relation skills.

Nice-to-haves
• 3 years of ambulatory care and home health care experience preferred.
