Licensed Practical Nurse Intake

Job Summary

A company is looking for an Intake LPN for home health and hospice services.

Key Responsibilities

• Manage the intake process for new patients, including referrals, insurance verification, and scheduling assessments
• Serve as the primary contact for patients, families, and healthcare professionals regarding services
• Ensure accurate documentation of patient information and collaborate on individualized care plans

Required Qualifications

• Associates Degree or Certification in Nursing
• Current Licensed Practical Nurse license in the relevant state or multi-state license through eNLC
• Preferred Bachelor's degree in nursing
• Two years of Home Health and/or Hospice experience
• Two years of experience in utilization review/chart audit
