Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans in collaboration with healthcare teams to support member transitions
• Facilitate education and resources for members and caregivers to enhance wellness and care quality

Required Qualifications

• Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing
• 2 - 4 years of related experience
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
• Registered Nurse strongly preferred
• RN Compact License/Multi-State License a plus
