Job Summary

• Accountable for all Care Management activities for assigned high and moderate members with behavioral health conditions
• Conducts in-depth health risk assessments and develops patient-centered care plans
• Oversees and manages the work of the assigned care support team to ensure quality care delivery

Required Qualifications, Training, and Education

• Master's Degree in Social Work is required
• Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN) certification is required
• 5+ years of experience in utilization management for mental health and substance health
• 5+ years of care management experience
• Proficient in Microsoft Office and typing skills
