Job Summary

A company is looking for an Admission & Discharge Registered Nurse.

Key Responsibilities

• Complete initial evaluations and initiate patient care plans
• Collaborate with client intake and recruiting teams for timely admissions
• Coordinate care with other agencies and conduct discharge visits as required

Required Qualifications, Training, and Education

• State of Indiana or multistate RN license
• Graduate of an accredited nursing school; BSN preferred
• Home care experience preferred but not required
• Experience with medically fragile clients is beneficial
• Ability to exercise initiative and independent judgment
