Home Health Care Coordinator

Job Summary

A company is looking for a Home Health Referral Care Coordinator.

Key Responsibilities

• Provide staffing for home health and IV therapy referrals and assist with new home health referrals
• Ensure accuracy of demographic data and obtain necessary documentation to staff cases
• Address inquiries and questions professionally, while promoting positive public relations with referral sources

Required Qualifications

• Some college education and/or medical background; bilingual preferred
• Experience with Work Comp Insurance, referrals, and authorizations preferred
• Knowledge of medical terminology and moderate PC skills, particularly in MS Office
• Able to work cohesively with coworkers and external customers with minimal supervision
• Knowledgeable of injured workers' rights and committed to maintaining confidentiality
