Key Responsibilities

• Conduct telephonic annual wellness visits
• Identify patient needs and create patient care plans
• Document patient interactions and collaborate with the care team

Required Qualifications

• Licensed as a Physician Assistant, Clinical Nurse Specialist, or Nurse Practitioner in Florida
• Experience in telehealth, wellness visits, and preventative care
• Willingness to obtain licensure in additional states
• Compliance with CEU requirements for licensure
• Proficient in managing electronic health record (EHR) systems
