Job Summary

A company is looking for a Nurse Practitioner - Transitions of Care.

Key Responsibilities

• Perform initial post-discharge assessments and medication reconciliations
• Conduct virtual urgent care visits to manage acute symptoms and ensure appropriate care
• Participate in interdisciplinary rounds to discuss high-risk patients and enhance team collaboration

Required Qualifications

• At least 3 years of experience in hospital, post-hospital, or primary care settings
• Active RN and APRN licenses in good standing
• Ability to obtain licensure in required states within 90 days of hire
• Active BLS certification at time of hire
• Experience in managing common hospital discharge diagnoses and post-discharge care
