Job Summary

A company is looking for an LPN Post Acute Referral Care Coordinator.

Key Responsibilities

• Process referrals for injured workers and obtain necessary clinical information
• Coordinate staffing and communication regarding post-acute care services with clients and healthcare providers
• Monitor progress of injured workers and facilitate discharges, ensuring all necessary services are provided

Required Qualifications

• Current LPN state license in good standing
• Minimum of one year of nursing experience
• Knowledge of injured worker's rights and confidentiality of Protected Health Information (PHI)
• Ability to maintain ethical and professional standards
• Experience with interdepartmental collaboration and teamwork
