Key Responsibilities

• Manage care services for clients in the Long-Term Services and Supports (LTSS) program
• Coordinate with healthcare providers and community resources to ensure comprehensive care
• Monitor and evaluate client progress and adjust care plans as necessary

Required Qualifications

• Must reside in Sarasota County, region 8
• Spanish speaking preferred
• Experience in care management or related field
• Relevant certifications or licenses as required
• Ability to work remotely and manage time effectively
