CMA Care Coordinator

Job Summary

A company is looking for a Care Coordinator.

Key Responsibilities

• Build rapport with patients through monthly telephonic outreach and care coordination services
• Assist patients with Chronic Care Management (CCM), Behavioral Health Integration (BHI), and Remote Physiological Monitoring (RPM)
• Develop and implement individualized care plans to improve patient outcomes

Required Qualifications

• Certification as a Certified Medical Assistant (CMA) or Nationally Certified Pharmacy Technician (CPhT)
• 2+ years of experience in care coordination or clinical healthcare roles
• Strong understanding of healthcare terminology and patient-centered care principles
• Experience working with EMR systems
• Intermediate to advanced computer skills, with the ability to navigate multiple systems
