Job Summary

A company is looking for a Pharmacist, Clinical/MTM Remote, PST.

Key Responsibilities

• Conducts telephonic medication therapy management and comprehensive medication reviews
• Analyzes medication profiles to optimize patient medication regimens and ensure safety
• Coordinates care and follows up with members and providers regarding drug treatment goals

Required Qualifications

• Doctor of Pharmacy degree or equivalent experience
• ASHP-accredited residency or 2-3 years of relevant pharmacy experience
• Active and unrestricted State Pharmacy License
• Continuing education to maintain an active pharmacist license
• Current knowledge in clinical pharmacology and disease management
